Go into your home folder, go to View, Show hidden files. Rename the .pulse folder to .pulse.old, then reboot. Once rebooted don't change any sound settings.
Now use the posting guide link from above to show us a screenshot of each tab from Sound Settings. Make sure Chrome is running and a Youtube video is playing in the background when you take the screenshot.
